Do You Need to Exercise to Lose Weight on Keto?

2017-07-31

This was a live video, and the question was the one I'd been asked most: do you need to exercise on keto? The short answer I gave was no for weight loss, yes for your health, and the order matters.

Watching this in 2026: the goal question

The line I'd lead with today isn't about exercise at all. It's my frustration that people posted "I'm trying to stay keto" without saying what for (video f119qGyJ7AY).

Keto is a tool. Weight loss, blood sugar, training performance and epilepsy are different jobs, and the right answer to "should I exercise?" changes with the job. That's why I split my answer: if being heavy is the bigger health threat, put the diet first; if you're at a normal weight and want to look different, exercise matters more for long-term health than the last few pounds.

That last minute also marks a turn in the channel. After this video I started making short, one-question videos built to be shared in threads, like the workout-carbs video.

Then vs now: workout carbs and walking

Two numbers in this video changed later. The carb dose for competitors, 20 to 50 g before, shrank to 10 to 20 g on each side three weeks later (video XhEJ9x5sP0g). And by the 2018 basics video I had a concrete activity target: if staying healthy is your one priority, 30 minutes of walking three times a day (video UygKGoP_BtA).

The core answer stayed the same in 2018: do you have to exercise to lose weight on keto? No. The best way to lose weight is in the kitchen, and meal prep comes first if you only have time for one thing (video UygKGoP_BtA).

The claim worth unpacking: "80% diet" and fitness as the top predictor

Both numbers in this video are mine, said live, without a source on screen. None of the evidence pages measures the diet-versus-exercise split, and none covers cardiorespiratory fitness and mortality. So I can't back up "80%" or "greatest predictor of death" from the pages here. Read them as my 2017 summary of what I'd read, not as checked figures.

What the evidence pages do say supports keeping exercise in the plan for the audience I mostly talk to. In the Health ABC study of 2,066 adults aged 70 to 79 followed for three years, those in the top protein group lost about 40% less lean mass than the bottom group (Houston 2008). The ESPEN expert group recommends at least 1.0 to 1.2 g of protein per kg per day for healthy older adults, with physical activity and resistance exercise alongside (ESPEN 2014).

And when you're eating less, higher protein helps protect what you've got: across 24 trials, energy-restricted high-protein diets gave less loss of fat-free mass (Wycherley 2012). Losing weight is in the kitchen. Keeping muscle while you do it is the kitchen and the gym together.

Diabetes and medication

I said in the video that I hadn't researched exercise with diabetes and wouldn't advise on it (video f119qGyJ7AY). That still stands. If you take insulin or another medication that lowers blood sugar, starting keto and starting exercise both push glucose down, and doing both at once is something to plan with your doctor.
